The Review and Flexible Response Team

This is a City Wide Team that carries out planned and unplanned reviews for adults with care and support needs.

Some of the specific functions include:

• Planned reviews of domiciliary care and support packages (including direct payments), residential care and nursing home placements;
• Unplanned reviews where there appears to be a change in needs (non -crisis situations).
• Quality Assurance and Risk of Provider Failure review, for commissioned services;
• NHS Continuing Health Care reviews and joint funded packages of support with ICB.

The Team also complete urgent crisis visits on behalf of adult social care connect via a flexible rota. The team role is to carry out an immediate visit to assess if a person is safe to continue living within the community with support. Adult Social Care Connect continue to retain responsibility for the case and for making any necessary care and support arrangements.
