Using these Procedures (Community Independence Sensory Services)

Southampton Community Independence Service Sensory Services is a multi-disciplinary team that consists of specialist workers who support people in their own homes and care settings to remain independent.

The team offers the following services to adults:

  • An in depth assessment of their needs;
  • Provision of appropriate equipment and adaptions to support independence, or advice about self-purchase;
  • Specialist orientation and mobility assessment for both indoor and outdoor mobility;
  • Specialist training in independent living skills;
  • Instruction in communication skills to support with either a hearing loss or visual loss;
  • Support to access a personal budget;
  • Functional low vision assessment and instruction in the use of low vision aids. Access to specialist Sensory Rehabilitation Clinic;
  • Individualised rehabilitation programmes for vision and hearing;
  • Registration for visually impaired and dual sensory impaired clients.

The procedures on tri.x will support practitioners to discharge the functions under Part One of the Care Act 2014 (including assessment, Care and Support Planning and review). Additional procedures and processes can be found in the Local Resources area or outside of this site.
