Using these Procedures (Learning Disability Team)

Southampton City Council’s Learning Disability Team provides services to individuals with a diagnosed learning disability with an IQ of below 70 aged 17 and over, and who have been assessed under the Care Act 2014 as having eligible care and support needs.

The team has the purpose of enabling Adults to develop and maintain their independence.

The Learning Disability Team utilise a strength-based approach to interventions to ensure best outcomes for the individuals they work with. The services provided include:

  • Completing Social Care Assessments;
  • Facilitating Direct Payments and Personal Budgets;
  • Protecting and Safeguarding Adults at Risk;
  • Developing Care and Support Plans with the Adult and those within their network of support;
  • Referring to Life Skills who provide support for individuals seeking employment;
  • Referring Adults for day opportunities including education and vocational;
  • Assisting with supported accommodation and placements;
  • Responding to carers needs;
  • Arranging respite provision;
  • Enabling the transition from Children to Adult Services for Young Adults;
  • Signposting or referring on to other agencies and organisations;
  • Providing information, advice and guidance.
