Using these Procedures (Social Wellbeing Service)
The key responsibilities for the team are Completing Care Act assessments and reviews for customers with complex health and social care needs.
Some of the specific functions include:
- Supporting adults who lack mental capacity to make decisions on meeting their care and support need;
- Supporting adults who lack mental capacity regarding managing finances and property requiring applications to Court of Protection for deputyship and appointeeship;
- Making applications to the Court of Protection for best interest decisions on health and welfare issues;
- Undertaking safeguarding enquiries for adults;
- Working where adults are seen to have additional/ high levels of risk. This includes participation in processes such as MAPPA (multi-agency public protection) led by the police.
- Working with people who are hard to reach;
- Reviews of adults discharged from hospital who have been placed in short stay residential accommodation where there are more complex social issues to resolve;
- Working with people whose housing needs are already known to social care where there is additional complexity;
- New NHS Continuing Healthcare Assessments;
- Risk of provider failure reviews if cases already allocated to practitioners within the Hubs
- No recourse to public funds (NRPF) with additional complexity (human rights assessment);
- Adults who are at End of Life.
